we are billing for outpatient facility, we are getting denied by certain insurance for codes a4550 and a4649, does anyone know if any insurance cover these codes, if we need a specific modifier. thanks Sue

Hard to tell without more info, but usually surgical trays and supplies are considered included with the other services.

I have used both of these codes successfully, also billing for an outpatient facility. You will need to send a list of the supplies you are including in this code to the insurance company with a letter of explanation. Not all will reimburse the code--even with the list and letter--but many will.
