Medicaid In Nevada

I just need some help - I have tried all the methods in billing medicaid; however, when I seem to get an answer from them they are telling me that I need to fill in box 9a: Medicare and to only bill one CPT code per CMS 1500 sheet, or they will tell me that the patient has to be QMB qualified - just more explanations and no payments.  I just need some help in making sure that my super bills are correctly filled out.  My question is what am I doing different than how I am billing Medicare (which we are getting paid on).  Any advise would help.  Thank you.
