What is the payment process procedure

I am in the creation stage of my business and I am writing my business plan and I do not know the best way to descibe how I receive payment from the provider after he has been paid by the insurance carrier. Can anyone describe to me how the payment process works; do I rely on the doctor to pay me or is there an ACH method to send the payment transaction to me?

That has to be determined by you.  How do you plan on charging your provider?  Will you be charging a flat fee?  A percentage? An hourly rate?  Your contract should spell out how you will be charging, when the provider will pay you, etc.
