You are correct about my post being wrong, I was distracted and put out the wrong information, and did not double check before hitting the post button. I sit here with egg on my face, and it won't wash off. 33 is a custodial care facility, and is why I used it as an example and I am used to using 11 as the facility on most of my claims, so I just wasn't thinking on that one.  Don't know where that box 34 came from? The movie I was watching I guess. Oh well, back to bed for some rest.

You may run into a problem with leaving box 32 blank.  I have seen claims rejected for not completing box 32.  When the POS is 12 we were advised that box 32 should be completed with the office address/info.  Some carriers require box 32 be completed no matter where the treatment was performed.  Some still accept it with 32 blank.  Just wanted to give you a heads up in case you run into a problem.   :)
