Medicare Psych Eval.

Good Evening,
I'm hoping someone can help me out.  I have a psychologist who is performing a psych evaluation for a bariatric patient.  The patients' primary insurance is Medicare.  I have been searching the Medicare website for some time now trying to find the billing guidelines for this procedure.  The procedure code will probably be 96101 with a dx code of 278.01.  Has anyone out there had experience billing this procedure/dx to Medicare?  Or can anyone send me in the right on the Medicare website?  I have been reading this forum for quite sometime and have learned so much-
