EP Modifier

Can anyone help me with a question I have about billing E/M code 99213 w/-25, if I am adding vacc. 90649 should I also add modifier -EP?

Are those the only two codes??  You shouldn't need the 25 modifier if billing 99213 & 90649 if there was a reason (diagnosis) for the 99213 (which there should be).  The EP modifier is a medicaid specific modifier and you need to check your state's Medicaid guidelines to see if it is appropriate for what you are billing.
