We have been billing 94664 with E/M code but it keeps getting denied.  All of the articles I can find on this say to append mod 25 to E/M code, which we are already doing, the E/M code is paid but 94664 is not.  Any help on this?

What is it being denied for?

It says it is not payable with E/M code

Michele:
Is it only one insurance carrier, or is it across the board?  Have you called customer service to ask for an explanation?  I would start with that.  It may be a plan exclusion.  The plan may not allow E&M's with 94664.  But I would ask for an explanation.
