Diabetes Education/Triwest

I work in an Endocrinology clinic.  One of our patients has been having diabetes education as part of her treatment with a CDE.  All of her education claims have been denied by Triwest.  I have been billing under G0108.  I use the educator's name as the renderring provider and our PA's or MDs' name as the billing provider (depending on who sees her that day). I am using a modifier 59.  Any suggestions?  I would really appreciate it!
