Where do I get the forms to file with the insurance company the change in Billing Agent? What information will I need to supply the insurance company to change this information? I just want to make sure that I am following the insurance company guidelines to make the changes. When I get a client what is the next step that I need to take to notify the insurance company that I will be doing the billing for the provider?

You only need to do this for Medicare and I believe it's form 855I

The form type will vary depending on the provider type I always use this link to obtain the most updated forms because they change periodically and Medicare wont accept out of date forms.
