We just released our 12th ebook "Pricing Your Medical Billing Service".  It's now for sale for $37 at www.medicalbillinglive.com.  We wrote about the four common methods of charging for your medical billing service with all the pros and cons of each.  Of course, we added several stories about our experiences in charging some of our providers over the last 17 years.  We wrote about percentage billing, flat rate billing, (and exactly how to calculate it) per claim billing and when to use hourly rates.  This 51 page ebook is filled with valuable information on not only how to charge for your services, but also what you should consider in setting  your prices.

I'm going to add my two cents here and no one has paid me to do so.. LOL   THIS is THE best book you can get on this topic. I know of two others out there and this one..by far tops them!!  GREAT resource!!!
