Clinic owned by Dr. S and an ARNP works their under the direction of a Dr Y (MD) twice a week at the clinic. Dr Y also has her own private office location which she works at the rest of the week.
PT / therapy services are being done on some patients. The ARNP sees the pt, completes the paper work "Therapy Treatment Plan" which lists exam finding, modalities procedures, systoms, neurologic complaints, specific goals. and the ARNP signs the form. The MD reviews this form in the patient's chart.

A. Dr Y tax id box 25, Dr Y npi in box 24J & name box 31, Dr Y provider info and group npi box 33a,
Dr S facility info in box 32.
who name and npi goes in box 17, 17A ?
(would it be Dr Y or the ARNP) or do i need anything in box 17 and 17a
