Confused about type of bill to use

they are a facility but only provide services for out patient.  They have been billing on the UB 04 before we took them over.  I thought we had to bill on the 1500 form also but they have been told by their insurance carriers that they have to use a ub 04 because they are a facility.   Which to me was strange because they are using the 90806 codes.

Again, it depends, are you billing the professional component or for the FACILITY charges?  That's how it works. Professional charges such as billing for the therapy is done on the CMS 1500  facility charges with revenue codes are billed using the UB form.
