I have a potential client who has an office/surgical facility for pain managment and anesthesia.  This is a new area for my billing practice.  For clarification, a UB-04 form should be used to bill claims for the surgical facility, correct?
I am also looking for the CPT code to identify CPT code 62311 is bilateral?  Everything I read says modifier 50 is not acepted and there is a specific CPT code, but I can't find it.
Any help would be greatly appreciated.

If I understand the procedure correctly, the injection could be on the left (modified=LT), right(modified=RT) or bilateral(modifier=was 50 but not accepted anymore).  How do you identify the procedure as bilateral epidural injection........
