P.O. BOX EXTRA 4 DIGIT CODE

GETTING DENIAL FOR ZIP CODE FROM MEDICARE - WE HAVE BEEN PUTTING IT WITH OUR P.O. BOX ADDRESS FOR THE PATIENT WITH THE 4 DIGIT CODE ALSO ON IT. ANYONE KNOW WHERE TO PUT IT ?  NOT SURE WHAT THEY WANT.

You need the physical address, not PO Box.

I was having the same issue. You need a physical address.
