I have billed claim to BCBS and got rejection,

99214 with 25 modi  493.90
94060                        493.90
94640                        493.90

I got paid for 99214 and 94060, but got denial for 94640 with "The procedure on this claim line has been determined to be incidental to another procedure performed by the same provider for the same dates of service."

Please let me know what i am missing in this claim so I can get payment for 94640.

use any modifier or diff dx as mentioned in the pt medical records.

There may not be anything that you can do.  The patient's plan may just not allow for those codes separately.
