How to find out the holder of a PTAN number.

Help!  My affiliation with medicare has been deactivated by the holder of an PTAN number.  This number is not mine.  How can I find out who is the holder to contact them and correct this mistake.   Medicare is saying I am the holder, but my number is different and I did not send any correspondence to deactivate my medicare affiliation. Consequently, I cannot bill and my claims are being denied

What you are explaining doesn't make sense.  If you are enrolled with Medicare then you should have an individual PTAN assigned to you.  THen that PTAN can be linked to another PTAN for payment.  The owner of that other PTAN could "terminate the reassignment" but only you can deactivate your PTAN unless you gave authority to someone else to do that.  If no billing is done then Medicare will Deactivate your PTAN for non billing.  If you call your local Medicare and give your ss#, PTAN and individual NPI they should be able to tell you if you are enrolled or not.  If you do not know your individual PTAN or if you don't know if you have an individual PTAN, you can call and explain that you don't know.  They will not give you your PTAN over the phone but they will tell you if you have one and how to get it.

Thank You, I plan to call tomorrow.  I agree it does not make sense.  I recently stopped working with a medical group and the problem with medicare is supposedly associated with this PTAN.  I do know my individual PTAN and NPI which is not the same.  I think this is an error on medicare's part.
