Technical and Professional components billed on HCFA 1500 form

I am trying to bill a facility fee for a Dr who does surgeries in his surgery accredited office same TIN for the provider and the facility. Does anyone know if I can bill facility charges and professional charges on the same claim and would I use modifiers 26 and TC?

Michele:
I believe you have to bill the facility fees on a UB04 and the professional fees on a CMS 1500.
