I am billing for a practice who does hospital consults. Most of the times docs dictates notes on next day of consult or may be after few days. Even though docs states the correct consult date in dictation, transcription line is taking the date of dictation as consult date. Medicare always ask for consult notes. So when there is discrepancy of consult date on the notes, we send addendum for correct consult date with docs signature. Until now Medicare was considering the addendums and were processing the claims but recently I spoke with 2 Medicare reps and they both advised that Medicare will not consider addendum and doc will have to redicatate the notes. As per me and my docs knowledge, they cannot redicatate the notes by law.
Any one is aware if doc can redictate the notes? Any help to overcome this issue is appreciated.
