Hybrid procedure billing

In a hybrid operating room, can a patient be billed for both the OR minutes used and the CPT codes for the procedures done?  For example an EVAR uses a surgeon as well as a cardiovascular physician.

Are you asking if the Patient can be billed or if the insurance can be billed?
