a patients comes to see the doctor.. no one referred him.. when submitting claims to ins do you need to also included the rendering dr info in box 17 and 17a or leave this blank.

I think it depends on the specialty of the provider. If it is a PCP or a family practice physician, Referring doctor may not be required. But if the provider is a specialist, it would need the blocks filled up.
