do i use modifier 59 when billing 97032,97140,97112,97110?
when i add the modifier do i add it to each code or only to one?

If they are each individually performed services you add it to each service.

it is for a provider that did therapy on a patient.

Yes, I see the therapy but is it for a Chiro or PT? They use the some same codes and mods. DMK is right. I download the medicare billing guides for the different specialty's and they give which codes and modifiers to use. Usually the other panels follow medicare/medicaid guidelines but not all. Good luck.
