CPT 99366

I bill for a private practice.  My providers want to bill for case management services. They are suggesting CPT 99366-99368.  Also, 99371-99373.  My question: Is anyone finding that 3rd party insurers reimburse for any of these CPT codes?  If we do bill for them, what is the fee for them?  I do not see any RVU's assigned to these codes.  Any help or suggestions will be much appreciated.

I have not billed for any of these recently but when we did they were never covered by insurance.
