new patient or existing

Doctor saw patient in nursing home.    A week later patient came to office for visit.   Do I bill new or exsting patient for office visit? 

Merry:
Existing patient. Place of service has nothing to do with this. If he or she has seen the patient within the last three years, that patient is an existing patient.
