WE USUALLY VERIFY BENEFITS BEFORE SURGERY CASES. LOOKING FOR A GOOD TEMPLATE LETTER TO SHOW THE BENEFITS AND ESTIMATED OUT OF POCKET THEY NEED TO BRING IN BEFORE SURGERY ?

A standard pre-D would work.

Pre-determination of benefits. Goes to the carrier with the CPT and providers fee. Carrier will tell you YES or NO the fee is within U&C.
