My physician saw a patient in the hospital and discharged her. Who does the patient follow up with? Is it with my Dr. or her own primary care physician?

Well, from personal experience, she should be following up with her primary care. Many Drs. can discharge a patient, if they happen to be on call.

Is your physician a specialist? I would say that it all depends on what the discharge papers say that are signed off by the attending physician in the hospital. Usually it will give instructions..followup with PCP or call my office for an appointment in "2" weeks for followup.
