Physical Therapy home visits (S9131) modifier

FirstMedicalBilling:
We got a denial from Amerigroup asking for a modifier for PT home visit S9131.
Does anyone know the correct modifier?. We tried using GP, CK but still receiving denials.

Is Amerigroup a commercial co or a Medicare HMO? Assume this service is being performed by a licensed PT. Is the patient unable to leave the home? Is the PT working with a Home Health Agency? Can you share the specifics?

I would go to their website and check to see what their code requirements and modifiers are.
I am unable to logon to the provider section but here it is https://providers.amerigroup.com/help/pages/login.aspx. You probably have the passwords etc to log in.
