ER PHY BILLING HELP 64450 & 12002

Marlena Gardner:
I am billing for the physician in the ER. I billed 99284-25 mod, 64450, 59 &51 mod, & 12002 59 & F1. I am not able to get any insurance to allow the repair or block. they are stating they are incidental to the E&M. Could someone help with this?
