For a clinic that provides physical therapy, PT, and services are provided by a licensed PT. Can the services by submitted to Medicare & Medicaid under the medical Doctor that is credentialed, as they review and sign off on the chart?

No, the services must be shown as delivered by the PT and the physician who referred the patient is the one who ok's the plan of care and signs the RX. The claim must show the referring provider with their NPI.
