New here. Need help with UB-04

Thank you Michelle and everyone else who responded. I purchased the book (thanks for the discount code :-) Found my first error right away e.g. type of bill!

For anyone reading this and you're new to the UB-04 form (but have experience with medical billing!!), save yourself hours of reading and research, just buy the book!!

Questions though, I'm billing for an MH/SA IOP so no admit date in Box 12 , right? (my second error)

Box 15- Patients are usually referred by a Sober Living facility, how do I answer this question? If it's a physician referral, do I then enter name and NPI in box 78?

Right, they are not admitted so there is no admit date.  Box is left blank.

I had to pull a copy of my book to see.   ;D  Anyway, this is not a field that should affect payment.  Sounds to me like maybe '2' for clinic referral.  It is just a 1 digit field.  I would NOT put the referring provider's NPI in box 78.  That is for "other provider" from the facility that is billing.
