Multiple authorization numbers

Hello,
I have a prosthetic claim that has 17 billing codes and I have been given a different authorization number for each billing code. does that mean I have to enter each code as a separate claim and send out 17 different claims ?

Wow, I haven't ever seen that before!  I personally would not do a separate claim for each one.  If your software allows you to I would enter a couple of auth #'s on a claim.  You can only include 6 lines on a CMS form so I would group them that way.  If the auth numbers are consecutive I would put a range.
