PPOM/Cofinity in Michigan

Hi-

I am working with a new clinic and am trying to find out if anyone has a fee schedule for physical therapy for Cofinity (previously PPOM).  Any help would be great.  I can't get the payor to send without going through the credentialing process. Thanks
