I am wondering if anyone has a sample letter for terminating an insurance contract.

You don't want to use any such thing. You need to follow appropriate guidelines outlined in your contract and/or with the carrier you have the contract with. If the contract does not have an opt out procedure in place, you want to contact provider relations and get the procedure for opting out for them.
