Does oral surgery get billed on cms 1500 forms?

Are you talking about the surgeon's charges? Or the facility? The surgeon's charges would be billed on the CMS.

Oral surgery charges get billed on the ADA form if you are billing Dental; however if cross coding to medical as required by some Dental insurances, then yes you would use the appropriate CPT and bill on the CMS form.
