Billing for a LSW

I need help again.
 
I have a LSW that will be working under our LCSW who is doing her clinical supervision, can and how would I bill it to the ins I'm billing in NJ?

The LSW has a NJ licences.

My LCSW has her cert for supervision first question I ask the office.
