Starting an LLC

Does anyone know where I can get the forms to fill out for an LLC online myself?  I have contacted a small business lawyer, and they charge way to much just to set up my LLC.

I would however consult a small biz tax adviser and make sure you get E/O... The LLC can only protect your personal assets but can be pierced in some cases. Go over the pro's and con's with a tax adviser/account.

I just recently used Legalzoom. It was so easy and they were very friendly and helpful on the phone. They filed everything I needed and I paid a little extra for them to check my city and the licenses I needed. They sent me a nice package with step by step instructions on what to do next. They will also get you in touch with a tax advisor and an attorney if needed.
