I'm thinking they want a type 2 NPI (organization NPI) in the billing provider field? Is this for a solo practitioner?

On the CMS 1500 Form (or electronic equivalent):

- Box 33a must contain Type 2 NPI Number (who gets paid);

- Box 24j must contain Type 1 NPI Number (who did the work).

Those two numbers must be different.

I can see where this might be true when a sole practitioner has no employees.  In such a situation, the provider has no need to obtain an Employer ID Number (EIN).  But my understanding is that, if the sole proprietor has obtained and EIN, he must obtain a Type 2 NPI Number connected to that EIN.

Can you clarify under what other conditions your quote might be continue to be true?  Or a link to a discussion that provides such information?  You've piqued my curiousity.
