Billing for (IOP) intensive outpatient programs

Have a question about billing for (PHP) Partial Hospitalization Program. The group treatment  programs are for five separate sessions in a day.  This is totally different than billing for regular 90853 with the restriction of one unit per day.

How do you submit the claims to insurance (with prior authorization obtained?)
