Hello- Can anyone tell me who I could contact at Anthem Blue Cross of CA to facilitate a change of mailing address for my newest client? I know this seems pretty simple, but my client and I have spent a great deal of time on the phone with Anthem's beloved automated phone system, and we have yet to talk to a live human being. Any contact information such as fax number or e mail address would be apprecieted. We have a change of address letter, and W-9 ready to go, but need to know where to send it.  :)

Here is a link to an address change form.  It provides the mailing address and a link to the provider's information at Anthem.  Bottom of page says the form was updated 02-15.
