If the doctor already have both type of NPI's 1 & 2 it means she has created an organization (any how by intentionally or by mistake) I don't think there is any problem in billing just discuss with her and try to explain her the situation and figure out if she created the organization or she registered type 2 NPI by mistake if it is by mistake then she must fix the problem and if she intentionally created an organization (thinking for future growth of practice ) then just file the claims with NPI type 1 in 24J (rendering provider's NPI) and NPI type 2 in 33A (Billing Provider's NPI). And if any payer deny the claim call them and to update their NPI records for the dr and organization (the procedure to update the NPI records with payer vary as per payer).
