Modifier 25

Have submitted modifier 25 with code 90837 and continues to be rejected.  Can someone advise why this modifier is rejected?  Is there a more appropriate modifier to use with the code?

Also is this for a 90837 with ICD-10? Some carriers are having glitches with this code and ICD-10
