Another thing to look at is how your provider is set up with workers comp vs how they are set up with Medicare and/or commercial insurance.  You may need to set things up differently in your billing software to get the correct ID number in the correct part of the form.

This happened to me all too often and what I started doing is requesting the name of the Supervisor or Branch Manager. I also noticed the adjuster's will respond a lot faster to emails, so I obtained the format of email addresses for all the insurance companies that I had claims with and i would send an email to the Supervisor and Branch manager and cc the adjuster.

This usually got a lot of things accomplished for me. I was top Work Comp collector at the company that I worked for not only because of my method, but also because I was very professional.
