Ins claim codes never seen before

I have been billing 12 years now and this is a new one. I recently received payment from an insurance company (UHC) and under all the payment information is the following information.
Other Claim Rel Identification  (1L) 45510
                                                (CE) Presbyterian Homes &amp Services
the remit also shows a claim # that looks to be from a primary payer. I'm glad its paid but its from the same payer as billed primary.
