Privacy Statement

What should be covered in my privacy statement? I am going to be opening up a medical billing business in my home.  I want to know what topics should be included in my privacy statement so that  I don't get into trouble with Medicare and Medicaid. Is there a website that I can go to that will help me write a privacy statement? If not would someone being willing to share a copy of their privacy statement so I have an example? Thanks for any and all help.

As a medical billing service we do not have a privacy statement.  We cover our HIPAA responsibilities in our contract and in our BAA.  We also have a Compliance Plan.  But no privacy statement.

I used https://www.freeprivacypolicy.com/free-privacy-policy-generator.php for http://medwave.io/privacy-policy. You're basically covering your butt for Data use and Privacy. I don't think you need anything special in the name of "medical billing or credentialing" within it.
