Does anyone do Gyne billing? my question is why is it some insurances will pay for a 99213-25 and a 99396 annual but some wont? how can I find more info on billing a problem with wellness? Thanks in advance

Is the 99213-25 billed with a medical dx & the 99396 billed with a routine dx?

Not really enough info to help.  Need to know the denial reason, etc.  Both codes should be allowed if billed properly.
