New Medical Biller needing help to bill a case management claim

Hi everyone.

I'm having trouble trying find the cpt codes that will cover nurse case management with the dx codes are related to auto accidents.  I have to bill to a HMO prior to sending the claim to the auto insurance because it's a coordinated policy.  Please advise.  Thanks
