FQHC / RHC

I am new to billing for FQHC / RHC.  I have a private practice primary care physician who has qualified for FQHC / RHC.  Would this be billed on a CMS-1500?  I assume since it is a PCP, it would be the 1500 but everything i find says UB04.   Is there a difference between FQHC and RHC?

We bill for a FQHC and it's done in the UB04 format, even though it is office visits.  I found an article that describes the differneces:
