I'm uncertain of the proper way to fill out a claim form when there are multiple patients such as with couples therapy. What I have been doing is putting one partner as insured in box 4 (whomever the subscriber of the insurance policy is) and the other as patient in box 2. Is there a better way to do this?
Also with couples therapy, is it appropriate to bill a 90801 at the first visit? Or should we bill 90847 only?

Usually when family or couples therapy is done one of the members is the primary person in therapy and services are billed under that person as the patient.  Usually the therapist will meet with both, as well as occasionally seeing them individually.  The therapists that I have billed for do use the 90801 if they are meeting with the patient the first time to assess/evaluate the situation.
