My physician (Family Practice) is billing a 76645 for a breast exam done along with a pap smear; but a 76645 is a breast ultrasound! I know there is another code to be used instead of this one, but I can't find it.
In addition to this isn't there a CPT code I can bill for a pap smear?

The 76645 cpt is definitely for the ultrasound of the breast so it should not be used for an office breast exam. There is not a code for just a breast exam. If the dr is billing for an E&M visit then it is included in with that. There are cpt codes for pap smears. They are:
Screening Pap Smear HCPCS codes: P3000 (technical)
P3001 (physician code if abnormal)
Diagnostic Pap Smear CPT codes: 88164 (technical)
88141 (physician code if abnormal)
