I am billing NY Medicaid for anesthesia, for pulling of teeth.  I am having a hard time getting these claims paid.  Does anyone know how to bill for them.  Do I need a specific diagnosis or modifiers, reports?  Some of these patients are mentally retarded.

What denials are you getting?
