Credentialing for a Physical Therapist

Hello,
I am new to this forum. I need some help. I want to know, how to do credentialing for a physical therapist who wants to do home visits under Part-B? I am a new billing service, so any help will be much appreciated.

You would need to complete a CMS855I application.  Section 4 E is for providers who will render services in patient's homes.  You have to complete the entire form along with the CMS 588 EFT & CMS 460 if they want to be a participating Medicare provider.

To add to Michele's response, do the forms through PECOS. You will have to set up a PECOS account online for the provider.
